New Patient Registration

Ask at reception for the necessary registration forms and when you have completed them, return to the receptionist.  You will also be required to provide proof of identification (name and address).  A combination of two of the following can be accepted:  Driving Licence; Passport; Local authority rent card; Paid utility bill; Bank/building society card/statement; Benefits agency letter/benefit book.

You will need to arrange for an appointment for a New Patient Medical with the Practice Nurse and with the Doctor if you are on any repeat medication from your previous GP.

The Practice will arrange for your medical records to be sent from your previous GP.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and provide proof of identification (name and address).   A combination of two of the following can be accepted:  Driving Licence; Passport; Local authority rent card; Paid utility bill; Bank/building society card/statement; Benefits agency letter/benefit book.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
