registrationNew Patient Registration

Ask at reception for the necessary registration forms and when you have completed them, return to the receptionist.  You will also be required to provide proof of identification (name and address).  A combination of two of the following can be accepted:  Driving Licence; Passport; Local authority rent card; Paid utility bill; Bank/building society card/statement; Benefits agency letter/benefit book.

You will need to arrange for an appointment for a New Patient Medical with the Practice Nurse and with the Doctor if you are on any repeat medication from your previous GP.

The Practice will arrange for your medical records to be sent from your previous GP.
