HOW DO I...

    

MAKE AN APPOINTMENT

Appointments can be made by either telephoning the surgery on 0191 3885600 or calling at the surgery in person. In order to comply with government targets for the NHS and Primary Care Access, we have been asked to work towards being able to offer patients an appointment with a Doctor within 48 hours of their request and a health professional ( Nurse Practitioner / Practice Nurse etc.) within 24hours. Emergencies will always be seen on the day of request.  To help us maintain this high quality service please let us know if you are unable to keep an appointment you have made.  Persistent failure to keep appointments may result in removal from the doctors list.

There are three types of appointment:-

"Same Day Release Appointments" - offered on the same day where possible

“Routine Appointment” – the next available appointment for non-urgent problems.

“Emergency Appointment” – a same day appointment for urgent problems.

SAME DAY RELEASE APPOINTMENT

If you ring the surgery at 8.00am on the day you require an appointment, we will offer an appointment on that day where possible

ROUTINE APPOINTMENT ( pre-booked appointments)

A routine appointment can sometimes be within 24 hours when the surgery is not too busy but it may be more than 4 days if you want to see a specific doctor or the surgery is very busy e.g. at holiday times, when the doctors are ill or during the winter months when we are generally busier. 

EMERGENCY APPOINTMENT

If your problem is urgent and you need to see a Doctor the same day you will be offered an emergency appointment with one of the doctors on duty.  If the receptionist is unable to give you an emergency appointment she will ask a doctor to speak to you on the telephone to assess your condition and advise on a course of action.

HOME VISITS

To arrange a home visit phone the surgery on 0191 3885600.  Please try to request visits before 10.00am for people who are unable to attend the surgery.  Visits will usually be made after morning surgery, if you feel an immediate visit is needed please let the receptionist know.

REPEAT PRESCRIPTIONS

You may telephone the surgery to order your repeat prescriptions on 0191 3871508 or by marking the items you require on the previous prescription tear off slip and placing the slip in the repeat prescription box which is situated on the front desk in reception.  Alternatively you may order by post, enclosing a stamped self-addressed envelope.  Please allow 48 hours before collection.  If telephoning your order where possible please ring between 10.30am and 3.00pm.  please note this service is for items authorized for repeat only. A prescription collection service has been set up by local pharmacies. If you require your prescription to be ready for collection at one of the named pharmacies, please see reception staff.

REGISTER

The practice area covers Great Lumley, Bournmoor and Chester-le-Street and will accept patients living within the practice boundaries.

The necessary forms to register can be obtained by calling at the Surgery where the Receptionist will be happy to help you.  You will be given the necessary registration forms together with a New Patient Questionnaire to fill in and requested to make a New Patient Medical Appointment with the Practice Nurse.  This is for a general health check to enable us to assess your health needs.  If you require repeat medication you will also be asked to make an appointment with the Doctor to have the medication authorised.

When making an appointment you can choose to see any doctors at the Practice and not just the one you are registered with.

OBTAIN TEST RESULTS

You may be asked to telephone for test results.  Please try to do so between 2.00pm and 500pm Monday - Friday on 0191 3871508.
